- Plan is not transferrable to any other patient, family member or friend
- No additional discounts can apply
- Plan cannot be used for services related to injuries covered under worker's compensation
- Plan cannot be used for referral to specialists
- Plan cannot be used for dental treatments in progress
- Full payment is due before plan discounts can be applied
- Plan is non-refundable
